I, the undersigned parent or legal guardian, hereby grant permission for my child to participate in
activities organized by St. George Chaldean Catholic Church.

I understand that participation in these activities may involve certain inherent risks, including the
possibility of physical injury. With full knowledge of these risks, I voluntarily consent to my
child’s participation and assume all responsibility associated with it. On behalf of myself and my
child, I release, waive, and hold harmless St. George Chaldean Catholic Church, along with its
clergy, staff, volunteers, and representatives, from any and all liability, claims, demands, or

causes of action arising from or related to any injury, illness, or damages that may occur as a
result of participation.

Furthermore, I grant permission for my child to be photographed and/or recorded during church
activities. I understand that such images and recordings may be used by St. George Chaldean
Catholic Church for promotional and communication purposes, including but not limited to
social media, websites, and printed materials.

I acknowledge that I have carefully read this waiver and release, fully understand its contents,
and agree to its terms.
